The evidence gap is measurable

A 2026 audit of exercise-induced cardiac adaptation research included 767 articles and 94,744 participants. Female-only studies represented a small minority, and women accounted for less than one quarter of the pooled sample. The audit addressed one research field, but it provides a concrete example of a broader problem: female-specific evidence can remain thin even when women participate widely in sport and exercise.

A methodological working guide published in Sports Medicine has also argued that menstrual status, hormonal contraception, pregnancy and menopause need to be considered deliberately when they are relevant to a study question. Complexity is a design problem to solve—not a reason to default to male-only evidence.

What twins can add

Twin research is useful because monozygotic twins share nearly all of their DNA and many early-life influences. Comparing sisters who later differ in activity, diet, treatment exposure or other behaviours can reduce some confounding that is difficult to address in ordinary observational research.

That does not make a co-twin comparison a randomized trial. Twins can differ in work, illness, medication, stress, relationships, sleep and many other exposures. Good twin research narrows uncertainty; it does not erase it.

Better questions for women’s exercise science

The strongest next studies should measure the outcome that matters—strength, power, sleep, bone, symptoms, cardiovascular adaptation or recovery—rather than blending them into one vague concept of 'female performance'. They should document reproductive status when relevant and avoid assuming an idealized 28-day cycle.
